The Aetna Golden Medicare Plan and Aetna Golden Choice Plan, Medicare approved HMO
and PPO Medicare Advantage plans, provide access to the Forever FitSM
Program. This exciting program for our Medicare Advantage members is designed to
help you feel fit and stay healthy. The Forever Fit program provides a benefit that
includes a free gym membership to any Healthways network fitness center. Plus, the
program offers you the opportunity to enjoy a wide range of resources that promote
exercise and a healthy diet.
Aetna's Medicare fitness program includes the following at no additional cost to
you:

FITNESS CENTERS
- An @ Home Pak (Physical activity kit) which includes resistance bands, exercise
ball, pedometer and instructional DVD. Call Customer Service at 1-877-243-3004 to
